- An initial screen for a person’s risk for malnutrition be conducted on first examination of a person with a pressure injury (e.g., upon admission to a hospital, at pre-admission visits for planned/elective surgeries, or upon intake to home care), whenever there is a significant decline in the person’s clinical condition, and when progress is not observed with respect to the healing of the wound(s).
- In assessing for risk for malnutrition, a validated and reliable tool should be used.
