Debriefing Tool

• All orders for restraints should be time limited in duration.
• The process of discontinuing restraints should include a debriefing process with the interprofessional team and the client/family/SDM to ensure that alternatives to restraint use are discussed and to contribute to a plan of care should restraints be indicated again in the future.
